• Entry deadline is Friday, May 16, 2025
• 30-Team Cap; first paid, first entered
• Entry fee is $468 per team payable by check, Venmo or Apple Pay. Team captains must send ONE in-full payment only as follows:

Check payable to Fourth District Bar Assn. and mail to:

                        Perkins Law, P.O. Box 1930, Boise 83701;

                        Venmo to @Angie-Perkins (last four of phone is -9897); or

                        Apple Cash to Angie Perkins at (208) 871-9897

• Entry fee includes green fees for six players, three carts per team, lunch, entry into the putting tournament, drink tickets, and a sleeve of golf balls per player.
• Lunch will be served to all participants after the tournament.
• The first round of the putting contest will commence during the tournament on the putting green after your team completes Hole # 3. Final round is after lunch, so stick around!
• Individuals who would like to enter without a team should email Angie Perkins (angie@angieperkinslaw.com) and she will attempt to match you with team. – No guarantees.
• Absolutely no outside alcohol permitted.
• ENTRY FEE IS PER TEAM NOT PER PERSON AND IS NONREFUNDABLE.
